The Status Report, is the main tool for monitoring a trainee's progress throughout the program. It also assists the trainee to plan their training and is used to provide formative feedback to the trainee from the Mentor and Director of Training. The report also provides confirmation of the trainee's status as Active or Inactive.

The six monthly training periods are:
The trainee is responsible for ensuring that the Mentor and Director of Training sign the report form, and that this is submitted to the Faculty office by the due date. There should be no exemption from this unless in exceptional circumstances.
Failure to lodge a report by the due date will result in the Trainee being considered inactive, and therefore unable to sit either the written or practical examination, or present at Ramazzini.
Failure to submit two consecutive Status Reports will result in the trainee being removed from the training program. Trainees so removed, who wish to re-apply to join the training program, will be assessed on a case by case basis by the Teaching and Learning Committee.
