Forms for Basic Training and Examinations
Following completion of the requirements of basic training and successful completion of the Written Examination and the Clinical Examination, a certificate will be issued certifying completion of basic training and eligibility for advanced training.

FORM   REGISTRATION INFORMATION
Annual Application for Approval of Basic Training
  • to be completed by trainee;
  • to be signed by DPT;
  • to be lodged with College Office during second or third years of basic training.
Application for the FRACP Written Examination
  • must be completed if the trainee wishes to sit for the examination in the year before taking the Written Examination;
  • eligibility for Written Examination is determined by CPT.
Application for the FRACP Clinical Examination
  • must be completed by those who have passed the Written Examination.
Annual Application for Continuation of Registration
  • must be completed for each year between the completion of basic training and commencement of advanced training (i.e. for each year during the period of undertaking the examination).
Annual Application for Interruption of Training / Extended Leave/ Withdrawal from Training
  • must be completed if trainees wish to interrupt/take extended leave/or withdraw from training;
  • must be completed if trainees wish to discontinue training.
