Applications are invited from suitably qualified medical practitioners eligible for registration in New South Wales for the above position within Central Coast Health at Home, Central Coast Local Health District (CCLHD).

Central Coast Health @ Home (CCH@Home) provides access, triage and linkage to community based health services for people in need of urgent care at home. This includes provision of care to people who are admitted to Hospital in The Home (HiTH) under a CCLHD Admitting Medical Officer (AMO).

The Visiting Medical Officer (VMO) for CCH@Home works as part of a team in providing care to people who are at risk of hospitalisation utilising both virtual care technology and face to face clinic appointments.

This is a key role in the provision of care to people in the community and supports appropriate and timely communication between the District, NSW Ambulance, Emergency Department, Hospital Inpatient Services, Residential Aged Care Facilities, General Practitioners and other key partners in the delivery of admitted and non-admitted community based health care.

Essential qualifications

Applicants should be a Fellow of the Royal Australasian College of Physicians or Fellow of the Royal Australian College of General Practitioners and/or other specialist recognition as provided for in the Visiting Medical Officer Determination.
