Of many wonderful experiences with patients, one illustrates the importance of making a diagnosis, and the interaction between colleagues with complementary skill sets. A farmer in his sixties experienced the progressive development of tiredness and lethargy over a number of months, which made coping with his farm work increasingly difficult. He thought old age must be galloping, but came to a haematology colleague via his GP. He was anemic and folate deficient - a combination that made my alert colleague think he might have malabsorption. He was right! The patient had developed late onset celiac disease. The response to gluten withdrawal was rapid, and I still remember the patient's delight that his life still had a future.
