About Evolve
Evolve aims to reduce low-value care by supporting physicians to be leaders in changing clinical behaviour for better patient care, make better decisions, and make better use of resources.
Evolve works with specialties to identify their Top-5 clinical practices that are considered low-value. Low-value care is clinical practices that, in particular circumstances, may:
- be overused
- provide little or no benefit
- cause unnecessary harm.
Evolve’s main objective of reducing low-value care practices also reduces related emissions and waste. This in turn helps to reduce future climate change impacts on patient health and contributes to building stronger more climate resilient healthcare systems.

Management of bronchiolitis
Have you read the RACP Evolve case study (PPTX) of Toby, a 3-month-old baby with a history of coryza and coughing? It reminds physicians that chest x-rays for children with acute lower tract infections are often not effective to distinguish between bronchiolitis and other lower respiratory tract infections. In mild cases, they don't offer information that is likely to affect treatment.
Bronchiolitis is a clinical diagnosis. Chest X-ray adds little to diagnosis or management of this condition.
Bronchodilators such as salbutamol, as well as anti-inflammatory medications such as steroids, have not been
found to have any positive effect on the severity or duration of illness in bronchiolitis, and could have
adverse effects.

Evolve Top-5 recommendations
Recommendations on low-value practices are developed through a rigorous, peer-reviewed process led by clinical experts, informed by evidence and guided by consultation and enable physicians to:
- safely and responsibly phase out low-value tests, treatments and procedures, where appropriate
- enhance the safety and quality of healthcare
- provide high-value care to patients based on evidence and expertise
- influence the best use of health resources, reducing wasted expenditure and the carbon footprint of the healthcare system.
