Fractional Specialist – Rehabilitation Medicine 15.2 hour per week (0.4EFT) – fixed term (Potential for ongoing employment) HM33 to HM41.

An exciting opportunity exists for a Rehabilitation Medicine Specialist to join our expanding Rehabilitation Team within Grampians Health. The team currently operates across our acute and subacute sites and consults to the Horsham campus on a fortnightly basis. Opportunities also exist within a range of community programs.

Key responsibilities include:

• providing specialist rehabilitation medicine care across acute, subacute, outpatient and community settings
• assessing, diagnosing and managing patients, ensuring accurate documentation and care planning
• leading multidisciplinary rehabilitation programs, coordinating care with allied health and medical teams
• developing and communicating patient goals, discharge plans and ongoing care arrangements
• supervising, mentoring and supporting junior medical staff while contributing to education activities
• participating in quality improvement, policy development and service planning initiatives

Essential qualifications

The successful candidate will have:

• qualifications MBBS or equivalent degree enabling registration with AHPRA
• medical registration in the State of Victoria and holding Fellowship of the Royal Australasian College of Physicians or equivalent where other qualifications are recognized by the Royal Australasian College of Physicians as equivalent or through College faculty membership
• recognised qualifications and training in Rehabilitation Medicine
• completed formal clinical training in Rehabilitation Medicine Demonstrated appropriate level experience and skills in the medical assessment and clinical management of patients
